Medical Records

Keep everything you need to process medical, dental and eye care claims in one file or folder. This would include copies of policies, instructions, blank forms, bills eligible for coverage, birth dates, social security numbers, etc. In addition keep receipts which are not covered for tax purposes.

When you move, request copies of your medical, dental, and optical files from your providers to take with you.

When a child leaves for college or to move out on their own, provide them with a vaccination history, X-rays, medical records, list of childhood illnesses, etc. This will be neccessary when medical attention is required.
